Changes needed in Form:

Form fields should be move up and down While creating Form.

Radio Button field should be auto filled for some values (ie. Yes/No) "Yes Autofilled”

There should be some field as conditional fields.

For IT

Logo is missing in Invoice PDF. Kindly check and do needful.
